Hi belle67

If you are taking the utrogestan continuously then it is very common to have bleeding or spotting during the first 6 months of any continuous combined HRT. Hopefully it will settle, but this will partly depend also on the dose of gel as well as the dose of utrogestan and whether you take it (the utro) orally or vaginally. On higher oestrogen doses and 100 mg oral progesterone you may find that bleeding or spotting continues - it just depends. Keep an eye on it and if it persists or there is a worrying amount of bleeding or you are at all worried, then do go to your doctor to get checked out.

Are you feeling better generally on this HRT?

when i took the pill microgynon 30 this had 30mg estrogen and 150mg levongestrel and i was fine on the pill no side effects but they won't put me on it as i'm 40 and smoke x

It contains 30 MCG (micrograms) of ETHINYL ESTRADIOL. Equivalent to at least 15 mg oral estradiol, in terms of hepatic effects, less so in terms of relief of menopausal symptoms such as hot flushes, about 1.5-3 mg oral estradiol.

Levonorgestrel is a progestin, somewhat androgenic. The content is 150 MICROGRAMS.

Erica28,  when you say hepatic effect do you mean SHBG-?

After being on birth control for a very long time I was put on HRT and it send me straight into menopause including all symptoms and it aged me in just a few weeks. No matter how high a dose of HRT (400 mg patches of 8 pumps Estrogel) symptoms (incl. hot flashes) just got worse and worse with no going back. Ethinylestradiol isn't working anymore, 1,5 mg of Estradiol, besides everything else, also gave me a beard on my chin and made me suicidal.

SHBG- is higher then 200
Free testosterone not measerable

So can you compare Ethinylestradiol with Estradiol as EE isn't bound by SHBG while Estradiol is? I feel no amount of Estradiol will ever come close to the potency of EE.

I am 52 with regular periods. My December period didn't arrive though( first missed period ever apart from pregnancies). I started on the gel for the first time around the time it didn't turn up. Was due to start the tablets today but it has turned up today. I don't understand why I can't just use the gel if my body is producing enough of its own progesterone to cause a bleed.
if i start the tablets now then I'll bleed again in 2 weeks surely?

erika28

hi erika28

so if i'm taking the estrogel how many pumps would i need roughly to balance 100mg utrogestan ? would this be 2/3

as i mentioned i was fine on the pill and suited me very well x

currently was using 2 pumps of gel but had lots hair shedding so have stopped it for time being been off it a month feel ok no different really less bloated and moody but hair still shedding but as i have no ovaries i'm looking at taking combined regime to see if it helps it may not i may feel worse but have to explore all avenues  x my levels of estrogen have varied over the year being 195 pmol after 3 mths then 6 mths 104pmol then 10 mths 368pmol one week and the following week 113 pmol as they missed off shbg test first time round but was 123.8 on second one with E being 113 and T 0.8 and Fsh 38

Hi Sammijane, I feel the same as you about getting the dosage right! I was on 2 pumps oestrogel, 100mg utrogestan and 1 pump tostran gel daily. I have never had my hormone levels checked. It definitely stopped my hot flushes after using for 12 weeks, but then got severe persistent thrush which I have had a weeks course of Flucanozole from my GP to try and get rid of it, so I have stopped all the hormones for now 2 weeks ago and will do for a couple more weeks until it settles and then try again. I never felt particularly marvellous so far while on HRT, but then have had the coughing winter bug for 6 weeks so that might have had something to do with feeling pretty rubbish!

Anyone else just restarted again after a few weeks?..my hot flushes haven't re-appeared yet at which I'm quite surprised, I thought they would come back straight away??!! Thanks

Hi Julia Dizzy - I think we've spoken before - that is a scarily high amount of testosterone.

Re stopping and starting HRT - I stopped for 3 months once (patches and progesterone) but flushes and sweats and other symptoms came back in full by this time so I restarted. They didn't come back in 2 weeks but started to reappear soon afterwards. It's fine to restart but I would restart the Tostran at 2 pumps per week up to a max of 3 which is the BMS recommendation. I would certainly also ask for T levels to be checked as they should be (as well as SHBG) by the clinic who prescribed it to you, private or NHS. Some effects of T take longer than 3 months so you don't want to build up to having an excess. I would also want to have oestrogen levels tested to make sure you are absorbing (as you are also taking T) to make sure your're not tipping the ratio of oestrogen/testosterone towards T dominance.

Yep I thought that Julia Dizzy - scary amount of testosterone!!!  My prescription and my label on Tostran says a pump a day but I know that that is the usual prescription for a man so I have always gone by what is said on here "pea size blob" two or 3 times a week max!  xx

julia dizzy

i've been 5 weeks of my estrogel and haven't had any symtoms really reappear other then couple of hot flushes at night no worse then prior hrt and vaginal dryness a little worse and sleep out the window but that's it , my hair still shedding so i think perhaps estrogel was helping it and maybe for me it's more an imbalance at play . my GP wanted to do a progesterone test to get a baseline and as expected this is low at 1 i believe or u set 1 my estrogen has dropped to 37 .
i've asked gp to do another blood test before i start hrt again to see what level testosterone is not on estrogen as i understand it taking estrogen hrt can cause testosterone levels to plummet so it would be interesting to see what mine are .
i honestly don't think i can go much longer without though although my mood has improved massively no ups and downs more even keel and aniexty gone so when i restart i'm going to try E and P .
i've also read regarding hairloss is that the hair continues to shed at the same rate but when u have low E this dosent stimulate hair regrowth which is why the hairs thins not sure if this is right but makes sense x lol

Thanks Hurdity, my tostran pump says one or two pumps daily as per meno clinic gynae Dr!! I shall heed your excellent advice though when I decide to restart HRT and space it out through the week. I shall wait a bit longer though to make sure the thrush has properly gone, don't want it coming back again, as I definately think the HRT caused it. My own Dr said I didn't have to be on HRT and it was my choice but the benefits outweigh the risks, but I do find it all rather frustrating.
I would like to try the Fem7 conti patches when they become available again?, Dr suggested those at my first visit to Meno clinic, but they were not available so he gave me the gel and utrog and said could go on those later on?.
I go back at end of Feb and shall ask to get my levels tested if he will (what is SHBG please??)

Sammi Jane I hope you get sorted and your hair loss settles down, it really is a minefield isn't it!!🤔

Hi Julia Dizzy - sorry - I always abbreviate as it takes so long to write everything in full so take short cuts so i can fit in as many responses as I can in a short time! Google will always tell you but here is one source: https://en.wikipedia.org/wiki/Sex_hormone-binding_globulin

Hi

Sorry if this question has been covered already. So, I used Utrogestan vaginally for the first time two nights ago.  Does any of the capsule or it's content escape the next day when you go to the loo?

Has anyone noticed this?

Thanks Jeepers
