Semi-continuous micronised progesterone routine?
« on: March 13, 2025, 07:40:37 AM »

Long story shortish and I could do with some more advice please.

I've finally been diagnosed with a over active thyroid via private consultant.   I've started an anti thyroid med (carbimazole) and we're working to getting the right level but it's a slow acting med and a see saw to get to the right place.

It's very clear that my thyroid symptoms are much worse in the week before and week of my period, I'm 55 but still quite regular (🙄).

One of the things my research suggests is taking progesterone for longer in the month to see if it helps.     Currently I take utrogestan 14 x 2 vaginally (and it's been a long process to get here!).

I'm thinking of trying a 21 or possibly 23 day regime to see if it helps.   Bleeding is the least of my worries currently so I'm happy to have a break.

I don't want to change progesterone at this time,  I'm trying to keep as much consistent to get the thyroid med level right.  I've tried increasing oestrogen that didn't help.

Could someone talk me through how to implement a longer regime?

A 25 out of 28 day cycle is typically suggested for this situation, ie take it for 25 days and have a 3 day break.

This is considered equivalent to continuous use, so you could take 100mg every night for 25 days.

Endometrial protection could not be reasonably assumed with a lower dose for only 21 or 23 days, so if you chose something like this you should really continue the sequential dose.of 200mg.

Thanks Bonbsh3ll, that's really helpful.   I can tolerate 200mg now (it's been a long hail) so that could  be an option,  presumably I could add some 200 days which would give me the added prog during the month?   The suggestion is that longer and lower might help the thyroid swing in getting.

My intention is to have a scan in the next few months as I struggled to get up to full dose for the first few months.

Thanks again

You have a lot of freedom as long as you take the full amount that you are prescribed per 28 day cycle.

So if that's 100mg for 25 or more nights, or 200mg for 12-14 nights, or something in between, that's fine.

What I would usually suggest to anyone affected negatively by hormonal fluctuations is just take the same every day rather than seesawing up and down.

You said yourself that bleeding is the least of your concerns, and sometimes that consistent stable dose of hormones makes for a better quality of life than chasing after your period.
