Utrogestan Dosage
« on: March 07, 2025, 11:53:11 AM »

I was on 3 pumps of Oestrogel per day but feeling awful with a level of 90pmol/l so increased my dose to 4 pumps.   Recent bloods show that my oestrogen level is still only around 250 pmol/l.  I understand I should now be taking 3 x 200mg Utrogestan cyclically for 4 pumps of gel. My question is if I’m not absorbing oestrogen well does this mean at 3x100mg Utrogestan my progesterone will be too high.  Feeling rubbish on 3, with horrible heartburn, bloating and constipation.

You have a lot of leeway with utrogestan because it's almost impossible to have 'too high' progesterone (as long as you are not one of the people who get side effects from it).

For eg - I take 500mg/day vaginally. 300mg at night and 200mg in the morning. My levels are still only just within the recommended level and no where near the top end of the range.

If your estrogen is only 250, you really don't need to worry about increasing progesterone though - unless you want to. But you should be getting it up beyond that. 270pmol is the bare minimum for bone health so you are not really getting the protective benefits of HRT. You might want to consider switching to 100mcg patches via your GP or going to a private menopause clinic to get prescribed above the licensed dose.

I am on 275mcg patches (2x 100, 1x 50mcg and a 25mcg - except at the moment none of that is in stock!) PLUS SIX pumps of Oestrogel. This gets me to around 650-700pmol. Ideally I'd be a bit higher but until the patch shortage is over I'm putting that off... There is no way I'd get this dose via my GP but she is happy to prescribe with a letter from Newson Health.
