Utogestan and gel

Hello,

I am post menopausal and have been on HRT for 9 years it was a life saver when I got it.

I started on Kliofem and was n that for a long time, I then moved to patches but had issues with spotting/slight bleeding and they did not stick very well.

I then moved to Utrogestan and the gel about a year ago and have had lots of issues of very heavy bleeding ...I've been moved on to 2 Utrogestan pills to see if this helps but I am still getting heavy bleeding and feel bloated, have awful wind and loose bowels since I increased to 2 Utrogestan pills from 1. I take them orally at night and I use 3 pumps of the gel in the morning.

Also I had a recent ultrasound and have some thickening of the womb that needs to be looked at...I am waiting to see my doctor...should I stop altogether the HRT ?? I saw a gynaecologist in March and she said if I stopped HRT my bleeding would stop.  Am nervous to just stop but now I have stomach problems with Utrogestan and concerns around the thickening of lining of the womb too...I feel constantly stressed and anxious....

I had a hysteroscopy done in March this year and that was all fine. But it is worrying that the recent ultrasound shows an issue...

Any help/advice please?

Hi Whattheheck,  I had a similar problem to you about this time last year it began.
Several years of different hrt and started on oestrogel and utrogestan. Seemed to be going well then everything changed quickly.  Bleeding, TVs and ultrasound...thickening of the womb lining.  Hysteroscopy/biopsy under GA. Finally after 4 months I was given the all clear.
They told me it would probably be best to stop all HRT until investigations were concluded so I did....but the investigations took so long that I never started hrt again.  10 months on I'm fine, I wouldn't say it's amazing but I'm ok.
I must stress this is my own personal experience and wouldn't advise anybody to do the same.  Maybe speak to your GP .

Im having the same issues at the moment.

Pleased to know your doing ok off hrt Northerngirl as I have a sneaky suspicion that will be my path now also x

My problems were the same as above.
Had biopsy which came back ok. Thank god.
But the sweats and other symptoms started to come back after only a month of giving hrt up, and tbh, it scared me. So I’ve started back on it.
My gynae said it was ok to go back on it if I wanted to.
So I’ve started back on 2 pumps gel and 100mg utrogestion. I’ll see how I go, as I was on 4 pumps gel before my problems started. AND I felt pretty good tbh on it. 🤷🏼‍♀️
If I increase my gel, I’ve to take 200mg utrogestion, which I’m not looking forward to, but it’s early days yet, I’ll see how i go.

You have my sympathy Whattheheck.
Why is everything so complicated! 😩

jillydoll Ive had to increase my utro to 200mg orally I was using it vaginally but mustnt have been absorbing it as monday i started with heavy bleeding.

I know its only been 4 days but its actually ok. I'll try and remember to keep you updated jillydoll in case you need to increase.

After having said all that i may have to come off hrt again altogether as i cant keep bleeding like this x

Thanks all for the helpful posts I feel am not quite so alone...  :'(

I am feeling rough on the 2 Utrogestan pills ....bad side effects mainly stomach related but not pleasant...

I also take Vagifem twice per week for atrophy as well as the 3 pumps of gel/2 Utrogestan.

My brain has gone to mush lately it is very scary I forget stuff from 1 minute to the next, and I used to have a photographic memory it is depressing!  I woke up at 3am as I couldn't be sure i took my pills before bed so I took another 1 so I may have taken 3 by mistake...am going to buy those pill containers withe the days on them as I am so forgetful now...

The gynaecologist told me I will need to stop HRT altogether if my bleeding doesn't stop it is horrible as I am getting it very frequently and very heavy...can't seem to catch a break.

you're definately not alone whattheheck.

my bleeding today is now on a different level. similar to what i had last year which was i wasnt absorbing the progesterome
 so i stopped the gel today no point in feeding it more estrogen x

I have been having similar. Post menopausal, 2 years on 2 pumps of oestrogel and 100mg utrogestan daily, all going great, felt so much better, then suddenly in May I started spotting. GP referred me for abdominal and transvaginal scan. I had to chase up the results and GP I spoke to wrote to hospital gynae for advice, which came back that they didn’t need to see me but to try reducing to 1 pump of gel. Within a few weeks my flushes and joint pains had come back with a vengeance so I upped back up to 2 pumps of gel, and the practice nurse said to up the utrogestan to 200mg each night. The only advantage I found with increasing the utrogestan was that I slept really well, but within a week had a period like bleed that lasted 12 days. I felt awful, I felt drugged, couldn’t wake myself up, really lethargic and miserable. When the nurse followed up with me the surgery did a referral to gynae under the 2 week pathway. I had a biopsy on 21st October now awaiting results, the consultant did a scan and said there was a slight thickening but nothing really worrying and he wasn’t expecting anything sinister to come back but discussed having a mirena coil fitted to see if that helped, so I had that done whilst still under the local anaesthetic from the biopsy. A week on and I’m now bleeding again but I know this can happen for a while until the coil settles.

I don't know much about the coil but I believe bleeding is normal when you first have the coil fitted x

Thanks, yes I was told it can take 2 or 3 months to settle. I’m just hoping it works eventually 🤞🏻.

Micronised progesterone is just not potent enough to achieve amenorrhea and endometrial atrophy in all users, and tolerability can become an issue at higher doses.

That's why for years the older synthetic progestins were favoured for use within HRT.

Don't feel like it is your only option, or be pressured into stopping HRT altogether just because a single progestogen hasn't worked out.

Little update am feeling very flat and very anxious :'( my doctor referred me to the gynaecology again at my local hospital under the urgent 2 week referral as I have had an abnormal ultrasound (few weeks ago) and lots of very heavy bleeding since August ...my doctor told me that I'd get a hysteroscopy done under a GA....I had one done in March and my doctor knew that she said no problem that is fine and another one can be done I felt re-assured.

Went to the hospital today and waste of time no real help and said that they didn't think i needed another hysteroscopy as my March one was fine and they believe that the HRT is the cause of the thickening of the lining, I told them that my doctor told me differently and referred me here to have it done but they are not doing it....they told me it s not up to the doctor's decide!

They had no real advice re my HRT am on gel and Utrogestan...they said I could try Femiston but that carries risks of blood clots as taken orally...am just defeated as go around in circles...I used to take Kliofem but stopped as safer ways to take HRT with no riks of blood clots.

I said that for my peace of mind I'd like a hysteroscopy done again and that is what my doctor requested that was her opinion when she saw me last week...the next steps so to speak....but gynae are saying no.  ???

Folks with bleeding issues (seem to be several of you)... the Mirena coil will almost always totally stop all unwanted bleeding. It lasts for 5 years once fitted.

No one has mentioned it here, so thought I'd point that out. It wouldn't be my first choice but if it was coming to 'stop HRT' or try the coil, I'd probably try the coil...

If you do have a hysteroscopy ever, they usually offer fitting it.

I've had 2 hysteroscopies and having a mirena was never mentioned 🤦‍♀️
Not sure I would have gone for it but I did think it was odd that it was never mentioned as lots of you ladies seem to have one or be offered it
Maybe it's a regional thing
