Starting HRT gel plus tablets - confused
« on: October 29, 2024, 09:52:39 PM »

Finally got some HRT gel + tablets but perimenopausal and still having periods. Do I just wade in and get started or wait till current period ends? Also don't really understand what the tablets say when they say take on days 1-25, like which days 1-25, sorry to be so thick  :D

Hi if you are peri menopausal then you take two Utrogestan tablets on days 15-26 of your cycle. The first day of your period is day 1. So you start on day 15. An easier way to do it is to start it on the same date every month or take it for two weeks, then two weeks without.

The dosage you have been given is for post menopause and is out of date advice.

Thanks, will speak to someone at GPs and/or pharmacy today. They really struggle to listen in the 2.5 minutes they've got for each appointment, not surprised they've not got it right!

I was prescribed 100mg for 25 days initially.  Then gp discovered he had prescribed incorrectly and put me on 200mg for 2 weeks per month.  So I took it 2 weeks on/2 weeks off, regardless of my own menstrual cycle.  You might bleed during the progesterone or after it.  Check with your gp if there was a reason they put you on a 25 day progesterone regime.

Not managed to get even a phone appointment yet, it was the 4th different doctor I've seen since I first asked but then my blood pressure was high so had a 24hr blood pressure monitor & then full bloods done. I think she's probably just assumed I'm post menopausal because of my age tbh. She didn't have much time to chat!

That's interesting Dotty, and I just read that on the NHS website.
I'm, 47 and peri and the doc also prescribed me 1-25.   I think utrogestan has contributed to my anxiety, but can't be sure.
Getting a coil on the 19th Nov, so I guess I will just carry on with the one tablet until then.
Great to be prescribed incorrectly!!

Hi Dorastar

It depends, as Dotty says - what your periods and cycle were doing recently in the months leading up to being prescribed hRT If you hearldy had any periods and they were spaced far apart - you couild try on the 1-25 and then you would get a bleed (potentially) during the 3 day break (or if you ovlated, at a different time).

However personally I would start with the 16/12 regime that is recommended/licensed for cyclical use which is 16 days of oestrogen only and 12 days of utrogestan 200 mg (days 15 - 26), where day 1 is the first day of the bleed. If your periods are far apart then no point waiting for a bleed and just start with the oestrogen  counting that as Day 1 and go on like that. You might have to tweak if your bleed consistently comes at the wrong time.

Then you can just get an appointment when you need to and ask whether there was a reason for the 1-25 utrogestan.

How annoying to have to keep phoning. In our practice we have online consultation (started during covid) so you describe the problem and then someone phones back later in the day - for a phone consultation (or to arrange a later visit if this proves necessary). Much more efficent way of dealing with things - and saves everyone time!

Hope this helps

Thanks, had periods in Feb, May, June, July, August, October so on & off really, been the same for a few years. Occasionally had 4 months apart but then back to monthly, could do either system I guess 🤷‍♀️

If you're still having fairly regular periods then you're probably better on a Sequi regime of talking the Utrogestan on days 15-26 of your cycle.  Or to make it easier....two weeks on and two weeks off.  So you take oestrogen all the time and take Utrogetsan for 2 weeks out of 4.
