Can anyone shed some light??

Good evening all.

After using the Mirena for many years for contraception I had one removed in February as it was at the five year point. I decided not to have it replaced as I lost my partner last year and didn’t see the need for any contraception. Slip forward to April/May and it became apparent I had lots of signs of menopause looming. I started on Evorel Sequi but the first round of the progesterone patch made me feel very anxious and my mood dropped daily so GP recommended having the Mirena replaced.

However the first night after having it put in I didn’t sleep a wink and over the following 10 days I felt completely wired, agitated, could keep still and anxiety went off the scale! I acytaully demanded they took it out that day or I was going to take it out myself! 🙈 Within 3 days of it being taken out all the side effects stopped!

This has left me very confused, how have I been okay for so many years with the Mirena and then reacted this way with a new one? My GP doesn’t seem interested in why this happened!

I’ve been given Utrogestan now and still scared to start HRT again incase it has a similar effect! Another GP has suggested I try the coil again and use diazepam to help while it settles. (I have diazepam prescribed for anxiety/panic and use as little as possible)

Anyone have any ideas as to why I had such a bad reaction?

It mayB that your hormones have settled enough that you no longer require replacement.  Perhaps ask for a referral to a dedicated menopause clinic, there R waiting lists both on the NHS and in the private sector.

Some people are progesterone/progestogen intolerant.

The Mirena gradually secretes a lower amount of hormone over time, with the first six months being the highest, when there is an increased likelihood of side effects. Maybe you were OK with it when you were younger because you had higher levels of your own hormones to counterbalance that.

You may tolerate Utrogestan better as it's body identical rather than synthetic, as is the case with  Evorel Sequi and the Mirena. You also have the option of trying it vaginally (off license) if you get side effects, which often helps with the sort of side effects you describe, as it is mostly absorbed locally, where it is needed. If not, you may need more specialist help

Such a shame that your GP is so dismissive but sadly, an all too common reaction, probably through ignorance.

I agree with the point about tolerating it better with estrogen on board - anecdotally quite a few women who struggle with progestogens find it less of a problem once on a therapeutic dose of estrogen, and in some cases testosterone can help with this too.

You would have had endogenous hormones in your system the last time you had one placed.

I would try the utrogestan first alongside your estrogen- it might suit you, and if not, at least you would be estrogenised if you decide to go for another IUS.
