Vaginal atrophy

I am new to forum. 71 year old with no children. Had menopause for 12 years from 49 to 61 when I took hrt pills. I have always had to get up once in night to wee.
Early in May I started rushing to wee sometimes not making it in time and felt a slight discomfort urinating. Due to lockdown I had telephone consultation with doctor who thought I had thrush although I had no discharge and minimal itch. One Fluconazole did nothing. Next telephone consultation suggested vaginal atrophy and inserted  Ovestin cream twice a week. After a few days it seemed a little better but after three to four weeks no real improvement and I went to see a nurse practitioner who tested my urine ....no infection sugar blood. A swab was taken from my vagina and nurse said everything felt fine. Low Thrush was result and I took one Canesten pessary and used cream twice a day.Symtoms still persisted. Another Fluconazole pill taken but again no improvement. I then saw a GP who suggested the thrush wasn't such a problem but believed VA was more likely. Said twice a week wasn't enough and suggested once a night ovestin inserted for two weeks and them reduce to twice a week. I am 11 days into once a night Ovestin and the tingling almost burning sensation has I feel increased. The burning is there more when my bladder requires emptying more than during urinating or finishing. If I am at normal rest bearing down produces a burning/ tingling discomfort. Can ovestin make things worse before improving? Anyone else have this scenario?

Hello craigowl, welcome to the forum.

It can take a long time to get VA under control, weeks or even months. As it is due to lack of oestrogen it will have been developing for some time before we get symptoms. I suggest you try and finish the 14 day loading dose if you can, and then drop to 3 times a week. If the stinging really gets worse you could try Vagifem instead, or the Estring. The latter is a silicone ring which stays in place for 3 months, and of course there are no fillers to irritate.

Regarding the bearing down producing a stinging sensation, there are a great deal of nerve endings in this area, so nothing would surprise me in that respect. I would suggest you try not to do that as it is not good for the pelvic floor.

Any local oestrogen treatment should improve the 'need to wee' symptoms eventually, as there are a lot of oestrogen receptors in the lower urinary tract, and VA can mimic UTI type symptoms.

Hi craigowl, I tend to agree with Joaniepat,I found ovestin stung like mad for me and dr switched me to vagifem,it seemed to do the trick,although set my anxiety off a bit,even though it's low dose but boy did it help with the needing to wee all the time,I'm down to twice a week on it now and things have settled,maybe ask for a change?

Hi Craigowl - I agree with what's been said already but would also suggest that you ask your GP for a referral to a pelvic floor specialist nurse. This will give you some answers re your bladder/urethra and what can cause the tingling and burning you mention. I have just had three telephone consultations with my local specialist nurse as I have had continued problems with bladder and pelvic floor following a hysterectomy for prolapse.  The advice has been excellent especially with dealing with the feeling that there is an infection present - so irritating and depressing. Are you carrying out any pelvic floor exercises?

I found Ovestin irritated me but the vagifem doesn't. I am currently on vagifem plus full HRT - I'm 66.

Hi  :welcomemm:

4 me it was like razor blades  :o up there.  After clear urine test results my GP started me on 'ovestin'.  VA needs managing.  Every night if necessary.

When I feel the 'need to pee' symptoms I take Nurofen, 3 times a day for 3 days.  Which eases those symptoms enough so that I can relax.

Some ladies need 'sylc' and/or 'yes' products.
