CarryWarry, please make sure the person you consult is a menopause/hormone specialist who is also knowledgeable on hormonal migraines.  You are getting into a very specialised area of medicine and you need expert advice.

Yes, thanks Mary.  I'm seeing my specialist again this week and also have my eye on Anne MacGreggor should this continue too much longer!  Two and a half years is long enough....will give one more go with my own specialist and then I'm off to Harley Street if no change.

I see Dr Newson she told me at my last appointment that you can't overdose on oestrogen and you can take as much as your symptoms require.  I'm on everol 100 patches and she said I can add gel as well when I feel I need it

As far as I understand if you take doses that are so high that your systemic levels become what is known as "supraphysiological" ie higher than occur naturally even at the highest point in your menstrual cycle - then a phenomenon known as tachyphylaxis can occur where symptoms return and higher doses have the reverse effect. John Studd talks about this on his website if you google it. Not quite sure how this works in relation to pregnancy where extremely high levels occur but I guess the whole hormonal environment in the body is different then so is a special case!

Also the dose you actually apply or take does not necessarily equate either to symptom relief nor systemic levels - this is very individual. Some women could apply 6 pumps of gel yet not be absorbing it and systemic levels be very low and still with symptoms - while others may find even 2 pumps is too much and achieve very high levels onoly on one pump. All HRT varies in terms of how much women absorb from each type.

So generally - if symptoms still occur then you need to increase dose  - but beware tachyphylaxis...this is my understanding anyway!
