Hi ladies,

Looking for advice on zoladex with tibolone?

I have now seen meno specialist Tony Mander again. I seen him some months ago and he advised I am suffering with severe pms. I had a lap done from another gynae and inserted the mirena coil.
I have had two mirenas before but this one did not agree with me at all, headaches daily with migraines, spots sore boobs and extremely moody and next minute crying my eyeballs out.

I got the mirena taken out last Friday and instantly felt a lift off me, been bleeding since but not the daily headaches. I have just started today like I am going to get a period, pains tiniutis louder and bit if migrainey type headache with the usual arm and leg pain.

Tony has advised that hysterectomy and ovaries best way forward for me and wants me to try the zoladex for next 6 months with tibolone added in.
What's everyone's thoughts?

I am frightened my symptons will get worse especially the tinnitus, this does drive me mad and think I am going to end up deaf because of it.

Willthe zoladex stop all my hormones?
Am I lacking oestregen and that's why I am getting all the symptons or is it hormone imbalance and the zoladex will stop it?

It's a big step and something I want if it's right

I am not familiar with either Zoladex or Tibolone and so I can't offer you any advice. I just wanted you to know that I am thinking of you, we are suffering pretty much the same symptoms so I can totally sympathise with you. I am counting down the days to my meno specialist appt on 26 sept and will let you know how what she recommends to treat my symptoms.

Hi mav,

How is the pill going? Is it cezarette your in.
I might try this before trying the zoladex.
I have noticed my ibs has come back now off the coil 😤

I just don't know what to do for the best.
The hysterectomy is what I wanted or hrt but Tony said hrt won't help me as it's pms.

Maybe that pill might?

Yes it's Cerazette......you could try that as it stops the cycle. It was dimming down my headache but I had to stop it as of last night as my eyebrow hair started to fall out again on it. My gynae did say before I started it that hair loss on it was very unusual as the progesterone in it was very low androgen so I guess I was just unlucky....
A hysterectomy sounds very drastic.....I think I would give Cerazette a try first....it's supposed to be stop periods within one or two cycles but it can cause irregular bleeding for some people.

Would you consider getting a second opinion via email from Dr. Currie on this forum? You could describe your symptoms and ask her whether she would suggest Cerazette, Zoladex/Tibolone or HRT....if Tony Mander is correct about it being severe PMS I wonder if you would benefit from perimenstrual Estrogen as a prophylaxis - you take a certain amount of Estrogen transdermally only in the days preceding, during and after your period to prevent the sharp fall of Estrogen....this strategy is dependant on having a regular cycle so that you can predict when to start the Estrogen.

I can't remember where you are in menopause ie whether you have become peri-menopausal yet? Maybe you don't know if you've had a Mirena as this can stop periods? From what I understand Cerazette suppresses the cycle as Mav196 says but it also depresses oestrogen levels so if you are peri-menopausal this is the last thing you would want I would say, if your oestrogen levels have started to fall.

I don't really know much about Zoladex except I gather it does close down the ovaries too to prevent ovulation - by eventually preventing production of FSH and LH.  Here is a link to some info on it: http://www.netdoctor.co.uk/medicines/cancer/a7798/zoladex-goserelin/ . The Tibolone replacement will give you back some of the this oestrogen and testosterone but I wonder if the oestrogen level would be high enough for you to feel good given your young age? The progestogen component of Tibolone would protect your womb. I am sure there are other options for example conventional HRT, after Zoladex, but I think the aim of this (the Zoladex) is to see if you feel better without your menstrual cycle and with hormone replacement, but isn't used as a long term option.

Good idea to maybe ask Dr Currie (consultation facility on home page of the forum) and the idea of oestrogen for part of the cycle sounds interesting - though not sure what happens about the additional stimulation to the uterus and whether you would need to take extra progesterone too?

Hi there- i have to be quick but wanted to answer. for me zoladex was not good- it plunged me into a depression and the tibolone didnt lift me out of it- i had to wait until the zoladex wore off to feel better. however, i do know it works for some women- i have pmdd but i also have severe depression during my period and the zoladex i guess mimicked the low oestrogen of a period and prob dropped it further so it was very wrong for me. but i didnt experience pmdd on it!! so i guess it got rid of that! be aware that- for me- the first month was part of the process of shutting down (tho paradoxically my system seemed to go into overdrive!)- the second month was when i definitely shut down. best of luck with it x
