Continuous regime and having more frequent bleeds + PMT
« on: June 15, 2023, 12:01:31 PM »

I moved to a continuous regime in February after being on HRT for 18 months, and still having a cycle. I understand from specialist that this is the current advice. Thing is, since then, I've had a very small bleed every two or three weeks, the bleeding I can cope with, but it's the emotions. I get anxious and unsettled for a few days. Will this pass and has anyone had anything similar?

Hi Winter,

What HRT are you taking? I find I get anxious and unsettled when my oestrogen levels are too high/progesterone levels too low. Also the bleeds are caused by high oestrogen building up your womb lining, which the progesterone is working hard to keep thin.

Hi Can you pls elaborate on bleeding which could be coming from too much estrogen and progesterone not able to protect lining?

Has anyone sought medical advice on this.

I am awaiting to speak to my doctor. In the meantime I get all these mixed messages from pharmacists who don't seem to know much.

Love to hear from you.

Bleeding for the first six months on a conti HRT is considered normal so try not to worry. My bleeding and PMT type symptoms carried on for longer than this and I reverted to a sequi regime as I found the predictable withdrawal bleed preferable to never knowing when the bleeding would happen.

IMO many doctors try to get you on a conti regime before your body is ready for it. Conti is supposed to be for those who are meno (1 year without a bleed). Once you're on hrt they make assumptions that may or may not be right. You may find you're better reverting to your sequi regime.

Hi Taz

Not sure how to respond directly to you. Thanks for responding to my concerns.  Just spoke to my doc and he now wants me to increase my dose from 50/140 to 50/250 Estalis Continuous patches. He said that increase in progesterone should stop bleeding.

Now as you mentioned that its common for 6 months, should I just wait to see what happens to see if I get further bleeds or just increase it, cos I'm concerned about any side affects increasing it. Did your specialist advise that its normal to bleed after 6 months. I've spoken to several pharmacists and they said it wasn't common and that to first do a pelvic ultrasound to check lining. But my doctor is confident its the medication and wants to try it first.

I'm thinking should I wait couple of weeks before increasing dose or just go for it and hope for best.

I'm such a worry wart, I haven't been sleeping over it.

Thanks so much. Glad for this site.

Ps. Interested if any other ladies are taking Estalis and their experience. xx

Did he say why you shouldn't be on the regime that has suited you for the last 18 months? If they feel like periods that's more than likely what they are and particularly as they're cyclical so it's less likely the scan will turn up anything abnormal. If you go on conti when you still have your own cycle it's very likely you'll bleed.

Hi GoGirl. I was so fed up with the bleeding when I reached the six month mark that I just asked my GP to put me back on to sequi as I got on well with that. I was still on that regime aged 60 when I had to have a hysterectomy due to prolapse. I hope you get some answers soon.
