I have a dreadful toothache that is getting worse. I braved the dentist yesterday and he said I had an infection that he thought was caused by debris getting into a gap between my teeth. Additionally, the tooth that hurts (which is next to the gap where the infection is) has a massive filling and shows an area of decay which he says can be refilled. He gave me amoxyllin.

I am not sure what is causing the pain, the dentist thinks it's the infection but the gum doesn't hurt it's the tooth. Presumably if its the infection the anti bis should eventually kick in and improve things, but if it is tooth decay I cant stand this for 2 weeks until I have a filling. Has any one any remedies? Im on back to back painkillers which aren't touching it, doing salt water washes and tried oil of cloves which made no difference.

Cloves or alcohol - it will dry the area which helps relieve pain.  ? osmosis ?  Drop a small bit onto the area that hurts.  It's the nerve in the tooth being open through the enamel. 

Get some 'nurofen' down you.  The ABs usually take 24 hours to kick in.  Do you have a treatment plan, I wouldn't bother with a refill unless I can see an X-ray of the area.  As we age gum retreat which allows debris to stick and infection to set in.  Get some Tipees to get between the gaps of all the teeth after eating to clear any debris away.

Oh Shropshirelass, I know how you feel. ABs should help and if pain meds not helping it could be trigeminal neuralgia. Can you get an emergency 🆘 dr appointment cos this pain is debilitating BUT sortable. A Google is not too scarey but will explain TN. Basically it’s 3 nerves (teeth, cheek and head I think) I’ve had it for 6 years now after being sure it was dental eventually went to dr. I’m on an epilepsy pill 3 x daily, a friend who also has it is on amitriptyline. Keep taking the pills and pain goes, miss a couple of days and it comes back 🥵.

Trigenimal neuralgia is characteristic.  It's a sudden pain down the cheek - caused when a blood vessel traps the trignimal nerve causing sudden, ACUTE pain.  It was so bad it would floor me, I'm sure that when I was walking the dogs over the fields that people thought I was drunk. I would hit the floor regardless of where I was.

There is an operation to release the nerve - long, involved and not many surgeons attempt it.  My Aunt had it twice with great success.  Amitriptolyne did nought to ease my sudden pain, it lasted intermittently for 18 months then suddenly went.  After I had intense dental treatment 3 years ago it returned ....... fortunately it didn't stay!

If the dentist suggests it's due to trapped debris contributing to a bit of decay then hopefully the ABs will help.

Its still pretty bad, I feel like I have been hit very hard in the face with a bass ball bat! I'm not worse though. Its been 48 hours since I started the antibiotics now. I'm getting stomach cramps which I guess is caused by the antibi's to add to the joy. Ive tried oil of cloves didn't help atall. I'm tempted by gin but think that could be a slippery slope. What Ive found works best is oragel it completely numbs it (horrible feeling initially) I always use tepe brushes as I have a big mouth and small teeth (as the dentist put it!) so have lots of gaps. There's always bits stuck after cleaning my teeth so I guess I must have missed a bit. Ive seen the xray, there's clearly a large area of infection between the two teeth but also an area of decay on the same tooth so don't know which is causing the pain.

Is the pain easing at all?  If not ring a Pharmacist in the morning for advice about pain relief.  Do you have an appt to return to the dentist?

Still not great, thanks for asking. On second set of antibiotics. it doesn't seem to be responding to them.

After the pain ramping up today I rang again in tears and they're going to clean it out tomorrow. Hopefully that will help, as I'm really struggling.

Oh you poor thing, I think toothache is one of the worst pains ever. My husband has had toothache recently, really bad.  He took Ibuprofen and Paracetomol at same time - was told that was safe by dentist so if yiu can take it maybe check if that’s possible for you.  He had a few really bad days but antibiotics helped , he’s since had a root canal and went through agony again but it calmed down and is now fine.  Hope you get some relief tomorrow, really feel for you x

Thanks both, not great news. He cleaned out the tooth and repaired an area of decay but didn't think either would stop the pain! He thinks that maybe one of the teeth has a decayed pulp. The problem is he's not sure which tooth it is and he doesn't want to treat the wrong tooth but the way I'm feeling I say take them both out! He suggested it may be TMJ, but I know it isn't as since it started I have a horrible taste in my mouth which is symptomatic of an infection / decay.

I have to go back for a review on Monday, if it gets any worse I don't know if I can stand it. Ive got some cocodamol so will try them with the ibprofen. The only thing he said he could do was to remove one nerve and if it still hurts to remove the other one so I guess two root canals.

Did he not tap each tooth along the row to see which 1 made you jump?

X-ray?  Get him to do both root canals at the same time? otherwise you will be back and forth too often.  Is there another dentist in the practice he could ask advice from?  Ours is a teaching practice to if there's a problem or technique the juniors are asked in to watch.

He did tap both and neither hurt when he did it! I had full set of xrays which were inconclusive. I think it must be because there is an infection under the tooth, or in the pulp which doesn't show up. My gum is swollen though.

Really strange.  At least you've had the tap test and X-ray.  My dentist won't do any deep cleaning etc. without anti-biotic cover if infection is suspected.  Have you been doing boiled, cooled, salted water as a mouth wash to keep infection at bay?  Yuk but it does help.

Yeah Ive tried everything, salt washes after food. Still got a couple of days antibi left, so Im hoping for a miracle! Not as bad as yesterday at the moment, which I'm very greatful for.

Have you tried any of the Corsodyl range of stuff?  I seem to be getting receding gums (have always had good strong teeth/gums).  Managed to get an abscess during first lockdown, I did manage to get antibiotics but also found the Corsodyl clear gel rubbed over the general area did help, it doesn't taste very nice and it says that over time it can stain your teeth, but when in pain...!  Hope it gets sorted soon for you.

Moneypenny, Ive been using the strong corsodyl mouthwash in the morning since it started to help fight the infection, or whatever it is. Not tried a gel, anything is worth trying and I don't care about staining atm, anything to relieve it. Thanks

I know the pain you’re suffering and maybe it’s worth GP having a look, they can tell if it’s TGN or decay before you get teeth out unnecessarily. Your 3rd last post was exactly what happened to me. I have a bridge next to the swollen gum and I thought it was going to break if my gum swelled up any more. My lip was swollen too as if someone had punched me!
 Apparently it’s called “the suicide disease” -  that’s what you feel is the only answer as it’s the worst pain on earth ! Please treat this as an emergency as you have suffered so long.

TGN is a sudden, sharp unexpected acute pain.  It would floor me.  In the nerve under the eye on the affected side.  It can be released by complex surgery.  It can leave a feeling of bruising along the sinus area. 

I agree, toothache and anything in the mouth is soul destroying.

I don't thik it is TGN as I have a foul taste in my mouth that only started with the toothache, which would suggest an absess or decay from what I've read. I hate toothache but TGN sounds so much worse as you can't get rid of it. Thanks for the good wishes, its nice to feel there are people out there when you're struggling.

I used to get a metal taste with sometimes a feeling that something was 'leaking' ......  hows the pain tonight?

Hi Shropshireless,

What you discribed could be a chronic infection and not an acute infection.  If so the antibiotics will probably have little affect.

 Route canal is likely to be the best option.  The dentist can deal with the infection by opening up the tooth and inserting a antiseptic wad, then resealing with a temporary filling.  Once the infection has gone he can then do the root canal.

Having the tooth removed is a big step so I hope the pain doesn't push you into that.

After a fairly good day yesterday pain wise, it's built up during the day today, after I went to the dentist for a review! The dentist is going to get colleagues to review my case and see if he is missing anything and I'm booked in for an appointment on Wednesday and I think he'll do the preliminary work for a root canal and hope its the right tooth.

Feeling a bit apprehensive about it. I haven't had very good experiences of root canals in the past.

Went back today and after talking about which tooth I thought it was (not really sure) and then him doing the cold test which seemed to confirm the one I though. He decided to do the prelim work for a root canal. Unfortunately when he started treatment the drilling caused extreme pain so he had to stop and he gave me more anaesthetic which did stop the nerve sensation. This all took a lot of time so he was unable to complete the root work, so Ive got to go back in the morning for that to be completed and to see if there has been any improvement in pain before him deciding whether the other tooth needs doing. Still hurting quite a bit, so I'm not sure if it's the right tooth. It's just such a nightmare. I couldn't stop shaking!

I've been thinking about you intermittently all day.  I've also been there.  The dental surgeon was unable to top up due to the medication I am taking  ::). 

Did the LA ease the pain from the area that you think the pain is originating from?
