Title: Hormone Blood test results

Anyone able to help with these Blood test results on day 14 of cycle;

Testosterone 0.4 nmol/l
Serum sex hormne binding glob 139 nmol/l

Free androgen index <0.3
Serum oestradiol level 155 pmol/l
Serum LH level 40 IU/Lj
Serum FSH level 48.9 IU/L

I'm on 100 Everol patch.  Minimal progesterone as am intolerant.

Am thinking I'm not absorbing? As feel absolutely rubbish!

Your testosterone is low and so is your estradiol.

However your SHBG is in the top quintile so possibly you are having higher estradiol levels intermittently.

The robust SHBG with the low total testosterone is resulting in a very low free androgen index.

However blood results are only part of the picture and can fluctuate massively in perimenopause.

How old are you and what are your treatment goals?

Thank you bombshell.  I'm nearly 54.  I've cleaned up my diet and this has made a massive difference however I am stiff suffering with progesterone intolerance and random heighten anxiety. Dryness issues which I'm wondering is to do with the low T.  I've got 13mm lining but have a monthly bleed so hoping that will reduce.  Feel so much better with E.  Since going gluten free and reducing processed food/sugar I have lost a lot of weight and wondering if that's why I'm not absorbing so well as I don't have much fat on me!

So tell Gp am progestone intolerant and she prescribes Medroxyprogesterone acetate non brand! 2 day for 14 days.....hmmm

Flamingo, I'm not surprised you feel terrible, that is virtually a post menopause level of oestrogen and your FSH is very high. 

Personally, I don't rate patches and I was never able to get a decent level of oestrogen out of them.  I much prefer Oestrogel and I like the dose flexibility too.  I really didn't like wearing patches either and they never seem to stick very well.  If you are using a 100mcg patch and getting a low reading like that then you are not absorbing it properly.

With that low level of oestrogen I doubt you need much progesterone.  I would have a good long run of oestrogen only before taking any progesterone and I would limit the progesterone dose too because of side effects.  If you are progesterone intolerant it's best to stick with a lower dose of progesterone over fewer days and have regular transvaginal scans.

I would think about switching to Oestrogel and you probably need start testosterone therapy too.
