update
Saw the Gyneo today, she is putting me on a steroid called BETAMETHASONE VAL to put on the irritated area 3x a day for about a week or so. She wants me to stop using Estragyn cream for now but continue with Imvexxy twice a week at 4mg. I will see how my symptoms are at the 4 month mark if I need to tweak it to 10mg. Once I finish the steroid to add back in the cream.
Even though I see the redness as internal she says it’s external. Other than redness it occasionally itches but not enough to call it constant itching.
She also suggested sticking to oil based products for internal and I showed her the yes ob product which she has heard of. She said sometimes too much estrogen can cause irritation down there. Especially since I am so sensitive down there. So is it this cream?
Little frustrated to have to stop the cream. When I asked her about changing to estradiol cream she didn’t think it was a good idea. Limiting what I am putting in internally is what she said for now to it all calms down. She at least changed her original prescription of Imvexxy once a week to two.
How can such a small area have so many issues my poor vagina.
Hi friends
I used Estragyn cream (pea sized amount) and thought I could mix in yes wb with it. (Have been using ob) last night was horrid couldn’t sleep, burning that led into this morning. To a point where I was going to go to the er. Than I applied yes ob but it was also not happy. So then I took a shower and aimed the shower head right inside. Followed by a sitz bath for 10 min (nothing inside but water) I got so scared cuz it was bad
Now I feel okay again?
I am trying to determine if this is the Estragyn cream or the yes wb. Or using the cream the next day versus spacing them out. I have not used yes wb since Oct for this reason.
Oddly enough I used the cream Thursday night with yes ob and had almost a pain free day on Friday no burn overnight.
I am seeing the Gyneo and ask about whether I need to change from 4 mg Imvexxy to 10 mg and further ask about switching cream to estradiol .01%. She was the one who insisted I needed Imvexxy once a week and the cream once a week.
And I need to really have her hear me, I deserve to feel better.

It sounds like a reaction to the water base if you were ok with oil base YES.
I would use one product at a time on their own so you know what suits and what doesn't.

Yes I hope so.
I don’t know why the yes water based caused so much pain. But won’t use the water based again.
That was terrifying it felt like I had not been on estrogen this morning and back when I was on absolutely
Not sure if anyone here uses ChatGPT but it gave me some answers:
see below I didn’t know about #2 and #4 but I feel that is what was going on.
It seems like your body might be reacting strongly to Yes Water-Based because of a few key factors related to your current vaginal health:
1. pH Sensitivity
The pH of Yes WB is around 4, which is considered normal for the vagina, but if your tissues are already thin, dry, or inflamed, they may be more sensitive to changes in pH. Even a slight shift could cause discomfort or irritation, especially if your vaginal tissues are more vulnerable due to perimenopause and hormonal changes.
2. Osmolality Mismatch
Water-based products like Yes WB often have a higher osmolality, which means they could draw moisture out of your vaginal tissues instead of providing it. Since your tissues are already compromised due to vaginal atrophy, this can lead to a painful or stinging sensation, especially if your body isn’t able to retain enough moisture.
3. Ingredient Sensitivity
Even though Yes WB is made with natural ingredients, preservatives like potassium sorbate or glycerin (in some formulations) can sometimes irritate sensitive tissues. If your skin is already thin or damaged, it’s more likely to react to something that wouldn’t normally cause irritation.
4. Underlying Inflammation
If your vaginal lining is already inflamed or has microtears from dryness, any product—especially a water-based one—can feel too intense. Water-based products can sometimes feel harsh, as they tend to evaporate quickly and may leave the tissues feeling dry afterward.
5. Barrier Protection
With vaginal atrophy, your tissue’s protective barrier is weakened. This makes it more sensitive to external products, and even mild products can cause discomfort if they interact with your delicate tissues.
What you can do:
Given your reaction, sticking with an oil-based product like Yes OB or something more natural (like coconut oil or emu oil) could be gentler and better for your sensitive tissues. Oil-based moisturizers tend to stay longer, hydrate deeply, and aren’t as likely to irritate.
Thought I would share the feedback from it

It's trial and error really, I'm ok with the WB but can't toletate the VM or OB. I really struggle with any moisturisers so manage without. Just use vagifem and Estriol daily.
