Title: Period erratic

Is this normal during Perimenopause?

I had a period on 18/11 which lasted about 6/7 days.
Then 10 days later after previous one ending I had another one which lasted a day.and stopped
I'm now bleeding again 4 days later.

I've 52 and been perimenopausal for 4 years could I be nearing Menopause?

I think U asked this previously  :-\ - for me my periods waxed and waned for several years B4 they disappeared.  I would have a few weeks of 'normal' cycle then miss weeks/months.  As well as the odd bleed which may/not have been cyclical. 

Menopause is literally the last monthly period.  We are advised that once we begin to miss periods to count for 12 months without - Nature however may throw a spanner in the works with an unexpected bleed so we have to begin counting from 1. 

How do U feel overall?

I feel fine apart from this, I'm on HRT and this and anxiety

Hi Jules 12. Which HRT are you on and how long have you been taking it? I don't think you mentioned that you were using HRT in your other thread which might change the advice given re unexpected bleeding.

I don't know what HRT regimen you are on but there are multiple bleed free options including the mirena IUS and some oral progestins that you could speak to your prescriber about.

These can completely eradicate bleeding even if you aren't yet postmenopausal, and can be used alongside your preferred estrogen therapy.

Jules, you need to give us more info. Like what HRT you are on, what dosage of P, orally or vaginally, sequential or continuous, what dosage of E, any blood test results you have....

HRT can alter how our periods act  ::).  Do U take anything for the anxiety?

Mine is the same.. I’ve had a constant period/spotting for 4 weeks now… and I’ve had the mirena coil for 14 months.. i’m so done with the constant bleeds.. they aren’t particularly heavy so the coil is doing it’s job to a degree as I was flooding before insertion.. but im not sure if constant bleeding isn’t more irritating. I praying im coming to the end and that’s why they are so all over the place

I'm on Femoston tablets 2/10mg tablets. I take one a day

If you're on Femiston, these aren't 'periods' because you are taking a synthetic progestin, Dydrogesterone. You take just estrogen for 14 days and then 14 days of estrogen plus Dydrogesterone. These are breakthrough bleeds, pills bleeds or spotting - not true periods - and as such can't tell you anything about your menopausal status. (Post-menopausal women can still bleed if they take hormones which trigger this.)

Because you are taking a combined product, it is difficult to adjust any of that. For eg, if you had a separate estrogen and progestin, you could increase the progestin. But you have a fixed ratio in this product with both E and P combined together. In addition, Dydrogesterone isn't available as a standalone progestin to add more of it in...

Is there a reason you are on oral HRT instead of transdermal? It's really only recommended now if you can't tolerate or absorb transdermal HRT. There is an increased risk of blood clots and an adverse effect on lipid profiles (cholesterol) etc with oral. In addition, you're on a synthetic progestin there - not body identical progesterone, which is now considered first choice and better/safer.

If you don't have reasons for being on oral HRT, you might want to ask to switch to transdermal (patch or gel) and utrogestan.

If you want to stay on oral, you could consider Bijuve which is body identical E and P although oral - and more P could be added with additional utrogestan P, if needed.

When i was prescribed HRT I wasn't given any options of which type, I was just automatically put on Femoston tablets

Jules, that isn't best practice anymore. Was it a long time ago? Either way, it might be time to find a menopause-aware doctor and get onto transdermal HRT and utrogestan/body identical progesterone.
