Title: Hello,I'm new on the forum....problem with spotting on HRt

Hello Ladies, I'm new here.  I have been on sequential HRT for 11 months.At the beginning on Evorel Sequi,for 6 months but not tolerate well norethisterone so my private consultant put me on Evorel 75 and utrogestan for 12 days 200 mg. All fine for 2 months and then 3 days after my withdrawal bleeding ends I started spotting for about 5 days. Contacted my consultant and he put me on 300 mg utrogestan  for 10 days. Again  2 months is fine not breakthroug bleeding,but when I finished my last period I started spotting pinky red 4 days later, on 14 days of the cycle now  and starting utrogestan tomorrow.Does anybody experience this kind of midcycle spotting ?

It can be normal for menstruating women to get a bit of spotting mid cycle as estrogen dips around the time of ovulation.

I used to get this from time to time.

If you are still ovulating, this could be the case.

It may also simply be how your individual body responds to cyclical HRT.

However as this is new and hasn't improved with increasing progesterone it would be a good idea to have a vaginal examination and a scan just to exclude anything that requires specific treatment. Also make sure you are up to date with cervical screening.

If everything is normal, changing the HRT can be considered for example if you are likely postmenopausal you could take the progesterone continuously instead of cycling. Also taking it vaginally works better for some - more gets directly to the uterus.

Alternatively a stronger progestin could be tried eg IUS or an oral progestin.

Hello, thank you for your response. I did a scan and all seems to be ok apart that I have 3 fibroids, one submuscular which is pushing on the endometrium, the sonographer said, that the endometrium looked fine and was 6.35 mm on the 14 days of the cycle and that I just about to ovulate. No polyps all other fine, fibroids have not grown since the last scan 6 months ago. I think that I still have a powerful my  own cycle on top of HRT. My period was regular and I never missed one yet. They vary between 22-28 days. Maybe I should lower the dose of estrogen, I CAN'T HAVE mirena HAVE IT 3 TIMES AND WAS EXPELLED due to fibroid location and the worst my my anxiety is back.I will be 52 in January and no idea in which state of perimenopause I am.
