Title: Evorel Conti patch no longer working! Can I add half a patch to make it work ?
Post by: Monsoon48 on June 27, 2024, 07:28:58 PM
I’m on Evorel Conti combi patch, I switched to this as I didn’t get on with Utrogestan 100 tablet. Also, at present I am having gynaecology problems so can’t have mirena.
I ‘ve been getting on really well with Evorel Conti, unfortunately for past 2 weeks, it’s stopped working! 😢
So now I’m back to sleepless nights, night sweats and hot flashes.
I did read that if HRT stops working, I need to increase dose.
So should I add another patch? Half a patch ? All feedback welcome
Thanks

MayB put the product name into the search box to read other threads, make notes.  I would add half a patch every 3rd night initially should I B in your situation to allow the uptake to kick in.

Half a patch at the most, a whole patch would be too big a change.

It's a shame the combined patch doesn't come in different doses but yes you can absolutely add another half patch.

There's no medical reason why you couldn't go up to 2 patches if you need to - plenty of women use a 100mcg estradiol only patch - but I would give the extra half a chance to work first, say 2-3 months.

If you were on an oestrogen only patch you'd go to the next strength up, so from 50 mcg to 75. Going straight to 100 is likely to cause you symptoms of too much oestrogen which can be just as debilitating as too little. It's unlikely you'd find a gp happy to prescribe an increase of this size in one go. Many women do need 100mcg, I'm one of them, but I spent two years on 75 before my symptoms returned again.

Just to add that adding extra half patches etc is off-licence though has been approved previously by Dr Currie on here. The only disadvantage is that you are then getting double (or half as much again) progestogen which may not suit some women.

The other point is that because the combi patches used for Evorel are the same dose of oestrogen and progestogen whether used sequentially (14 days over 28 day cycle) or continuously, theroetically, for some women, adding a small amount of additional oestrogen MAY provide sufficient protection for the uterus, though if this is adopted for continuous combined HRT, should only be attempted with small increases in oestrogen only  on top of the Evorel conti) and under supervision of a specialist who can monitor the endometrium thickness. This is more likely to be approved in cases of progesterone intolerance, I imagine.
