I am 61 postmenopausal for 18 years. Have interstitial cystitis and adenomyosis so a bit complex. Use Femosten 1/5 and Vagifem but for years have had bleeding problems with a number of scans and hysteriscopies. Tried other regimes ie mirena and utrogestan and patches which flared my bladder. Decided to see a specialist for advice. Where do you go? It’s so confusing

Your GP should be able to refer you to a dedicated menopause clinic, either NHS or private. Sadly, both have long waiting lists.

Definitely worth asking GP for referral to meno clinic (nhs). Slow but worth getting in q.
If private is an option for you, I’d recommend finding a consultant (not private Gp who charges consultant prices) gynae who specialises in (as in mostly does) meno, not a general gynae even if they list meno among their many interests. Most don’t need Gp referral. Some do have long lists (less long than nhs) but even the best have colleagues/teams with shorter waits so you could see someone more quickly. To choose someone, browse around here, search on your priorities, and ask around.
