Title: bleeding do I stop Utrogestan or keep going ??

Morning Ladies

I have been using Utrogestan every other night for a few months. This weekend started with heavy period, clots and really bad period cramps. Do I stop Utro whilst bleeding on insert during my period?? Not sur what to do :-\

Why are you using it every other night? Isn’t utrogestan taken either daily or for a certain number of days each month?

my gyne said 100 every other night vaginally  is acceptable and better for side effects. I expected a bleed as only been using it 2 months but my question is do I stop whilst bleeding on carry on inserting during a bleed.

Are you meno or peri? If meno I'd be a bit worried this dose is insufficient to keep the lining thin, you might be better to go with the manufacturers recommended dose of 100mg every day. If you're peri and this is your own period perhaps you'd be better with a sequi regime. I'm not sure so it matters too much what you do for the next few days. If I was in your shoes if meno I'd continue taking it but if peri I'd stop now and go on a sequi regime. Sorry this may not be very helpful.

The advice of Sheila is spot on. You either take continuously or sequentially. Anything other, could potentially cause erratic bleeding.

If post meno, you probably need to continue taking it.

I am post meno age 64 sensitive to all Progesterone. Gyne said every other day vaginally will be ok.... I will carry on for a while.

When i seen my gynecology last year she told me i could try taking 100mg ultrogeston every other night vaginally but when spoke to my gp she said def not so who are we supposed to listen to

Hello ladies.

Warwick01 - When I was in a similar position I continued to use the Utrogestan though I had no idea how effective it was.

At the time I was post meno and my Dr at Newson Health told me to use the 100mcg capsule vaginally every other night. I was also advised to gradually increase my gel and over time my breasts became sore and I had some spotting. I always felt that the Utrogestan wasn't really doing the job and then one night I accidentally missed a dose and from then on I had a bleed. As I say I continued with Utrogestan but I then changed regimes and moved to Cyclogest pessaries.

All this was about eighteen months ago and I no longer have any bleeding although I have also reduced my gel by a large amount. Fortunately a check up revealed no uterine problems and a very thin lining.

I was never happy about using Utrogestan vaginally, especially on alternate nights and with a hefty dose of gel. It didn't take much for a bleed to start and all the spotting was probably an early warning sign. Having said all that perhaps the Utrogestan was doing okay because apart from the bleeding my uterus was in good shape!

A specialist can prescribe off licence but a gp usually can't. I've always used the licenced dose because I'm not sure how much testing has been done with the half dose. It's more often prescribed by private specialists and their patients probably have scans to make sure the lining isn't getting too thick. *probably* it's OK with a low/medium (2 pumps) dose but maybe not with high doses. The only way to know if it's doing it's job is to have a scan but breakthrough bleeding may well be a sign that it isn't.

I think the jury is out on the half dose utrogestan. If only it came in 50mg doses so it could be taken every night for those who are very sensitive to it. My GP is fine with alternate night of utrogestan though that is not my regime. There is research to back this approach once a woman has reached menopause.

But many women find it doesn't work for them. In your position I would either take it orally (though that may not suit you) or stop it for a few days until the bleeding stops.

The other possibility it to double the dose vaginally to see if it stops the bleeding. Rather messy but not dangerous. Just make sure you keep pelvis tilted for about 30 mins after inserting so it absorbs (it absorbs quickly) or insert a tampon to keep the tablets in. Depends whether you can be bothered!

I take utrogestan vaginally every other night with Evorel 25. A week ago I cut a slither off the patch in an attempt to reduce the estrogen a little. Did it because, although my symptom control is fine, I need to find ways to reduce the horrendous fluid retention estrogen causes to my body. But I’ve started spotting and now a proper bleed. Which links to the original thread question - when bleeding occurs on HRT, what is it most likely due to? Progesterone or estrogen? I’m continuing with the utrogestan regime despite proper bleeding for four days now, as I figured that the body still needs it.

Can tweaking a low dose estrogen patch cause such a big effect?

In my experience the cause of bleeding has always been too much Oestrogen. Whenever this has happened it was due to me increasing my dose and then resolved when I lowered it again. I was three years post meno when I started HRT so perhaps that is a factor.

Jillyboo - it worked when I was on estrogel but doesn’t seem to work with the patch! Strange….

How did you resolve it? I definitely can’t take Utro orally every night because I tried it before and became a human sauna.

Hi Kathleen - see, this goes to show how different we are. Because I’m taking just less than the 0.025 patch which is mega low oestrogen. In my case, it’s likely that the Utro needs tweaking but haven’t got a clue how - here goes another £350 to spend on consultation fees dammit  :-\

How long have you tried it for?  You may find your body adjusts given time.

Jillyboo - I tried taking it every night (orally) for around 3/4 weeks then had to stop as I couldn’t bear it. I haven’t taken it vaginally every day though???

Utro never made be spot or bleed whilst using the gel, no matter how much I tweaked the gel. There’s something about using a patch that makes the utro a bit hit and miss for me?

In any case, definitely going to have to make another appointment as I’m close to giving up HRT. The fluid retention I’m getting with oestrogen is horrendous, which is the other issue I need advice on. xx
