Title: Can I increase Utrogestan to stop breakthrough bleeding

I have been on many different HRT and thought I had finally sorted it. I am on everol oatches and Utrogestan continuous. Have been on this since august. Started on everol 50 and 100mg Utrogestan no problems for first few months then I got sweats etc back so increased to everol 75 all good again until last month had a small bleed that stopped all ok again. But it’s started again. I’ve previously had scans and hysteroscopy etc and all ok it’s definitely HRT causing bleed.

Has anyone increased their Utrogestan to 200mg a day continuously? I know I won’t get to speak to a doc for a while so thought I would ask on here? I’m sure I’ve read somewhere that by increasing the dose will control the bleed.

Have a look at p6. The BMS suggests increasing the progesterone of bleeding is a problem . x

I just had a bleeding due to increase of patches from 50 to 75 and the scan showed all clear. The gynaecologist advised to up my Utrogestan to 200 mg a day (continuously) for one month at least. My bleeding stopped prior to the scan.

Hello Christine 49 and welcome to the forum.

I have a had a few experiences of post meno bleeding after increasing Oestrogen and after investigations I was told to reduce my HRT.

I think your idea of trying an increase in Utrogestan is a sensible one as this may stop the bleeding but allow you to carry on with a sufficient dose of Oestrogen.

Thanks for the replies. I think I’m just worried about overdosing on Utrogestan!! I will try it for a couple of weeks and try to get an appointment.

My last blood tests showed low estrogen on the gel as it wasn’t absorbing so I was put on the patches at 50 but increased to 75 as flushes etc were still happening. So all that has stopped but bleeding has started so I am wondering if it’s an estrogen dominance now as the patches are working well.

I assume you're meno not peri? Increasing to 200mg is a known strategy to control problem bleeding. If you're peri rather than meno you may need a mirena if you want to be bleed free. The other possibility would be to use 100mg for 25 days per cycle. The 3 day break is to allow a bleed if the lining has built up.

I’m sure I’m meno. When I had a 10 month break from HRT I didn’t bleed. Only seem to bleed when on it!

Sod's Law  :(. You're not alone,this does seem to happen to some people. Some of the synthetic progestins seem to be better at controlling bleeding than utro but of course they're not body identical. If nothing else works a mirena should sort it out.

Recent guidelines from the British Menopause Society are for women on 75mg patch/3 pumps of gel or more, to take 200mg continuous utrogestan or 300mg cyclical.  I would speak to your doctor about increasing your utrogestan to 200mg continuously.

I was on ultrogeston 100mg and oestogel for 7 months and  was bleeding all the time my gp upped my ultrogeston to 200mg unfortunately for me it didnt make any difference i still bled, so aftet seeing gynecology i was advised to change hrt im post menapause,

Has increasing progesterone helped with the bleeding as I am currently in the same situation as you but not increased as unsure 🫤

Me too...spotting ..have decreased estrodot patch by snooping end off as doctor is rubbish ...says it's up to me what I do ..thanks ..and referred me to gynae ...and long wait where I live 😫
