Hi all, I’m 52 and have been having hot flushes , initially severe 4/5 an hour everyday and night sweats for all of 2020. I tried various “natural “ remedies , oestrogen rich food, soya, supplements, exercise including strength training, cutting out alcohol! Anyway I was very sceptical about HRT and nervous, if I’m honest. But after having disturbed sleep for over a year, constant embarrassment in the work place and reduced to now no libido, I thought I would try HRT. I read untold articles and watched YOu Tibe reports and act felt quite confident in my decision ( really struggled to get any support from GP). Initially started on a tablet but after reading that it was post menopausal women (I had a period within the last year, albeit irregular) I came off it and requested a patch. Started on Evorel 25 until I had my Mirena coil removed Apr 21 (after 7!years and in place due to Fibroids) . Still incurred hot flushes throughout although reduced. Put onto Evorel Sequi, (Evorel I believe?) had various bleeds which I had not had for around 8 months. Had various investigative procedures to elongate in explained bleeding ( as directed by GP) including a hysteroscopy)… all normal! Asked Gp to increase the HRT to Evorel 75 as still have disturbed sleep due to hot flushes although daytime ones only a few now,with a top up progesterone. GP has had to e mail gynaecologist each time and now referred to a menopause clinic with the appointment next February! GP has now prescribed Evorel 75 with a top up Progesterone tablet(Ultrgesterone, 1 daily for 22 days), have been experiencing heavy bleeding since, losing blood clots every hour since!! I don’t know what to do as the GP seems quite dismissive for all the appointments I have had over the telephone and just wants “to pass me on”. I feel really unsupported AbS don’t really know where to turn, should I just wait to see if this new patch settles down and the very heavy bleeding stops ? Please advise. Thanks

Hi not sure why the gp has given you Utrogestan for 22 days. It should either be 100mg daily or 200mg for 12 days every month.

As you were using a Mirena coil it’s hard to know where you are in menopause. It could be that you are still in menopause and still having periods. Evorel Sequi is a sequential HRT designed to give you a monthly bleed, so the bleeding you had on that is to be expected.

I don’t quite understand what the GP means by a top up “progesterone”…. Every woman with a womb needs a progesterone to protect the womb.

I think the best thing would be to take the Utrogestan on a sequential basis…. 200mg on days 15-26 of your cycle. That should regulate the bleeding.

Thankyou so much for responding.The Utrogestan I’m taking is 100mg, 1 tablet ( apologies it’s for 25 days) . I have no idea where I would be in my cycle as hadn’t really had a proper bleed since Feb, other than the breakthrough bleeding whilst  in the sequi. I’m just concerned that since starting the Evorel 75 I have been bleeding for nearly a week and as mentioned, very heavy, losing clots. Is this normal or has anyone had these issues?

Hi when you start any new HRT bleeding can be expected for the first 3-6 months. x

Thankyou, this has been very helpful and re-assuring. I didn’t want to keep bothering the Doctor , as J feel like I am wasting their time and it’s almost like they “are getting cross with me” but that could just all be in my head!! You seem to ‘know your stuff Dotty’ so does this dosage sound ok, I have followed the instructions from the leaflet supplied.

Also apologies for all the typos!! This often happens when I am using my phone and don’t proof read  :)

Hi the dosage of Utrogestan is 100mg every night ( that is up to date guidance ). Anyone can take it this way but if you are still in peri menopause then you are likely to get breakthrough bleeding.

The dosage for peri menopause is 2 capsules on days 15-26 of your cycle. Day 1 is the first day of your period. This should give you a regulated bleed. x

Yes, changes are scary , I guess if option A,B,C aren’t working then we will try D. There’s so many options out there for HRT to I guess it’s just a case of researching all the options available and trying then h til one works ☺️ The bleeding is a worry though when it is large clots. Has anyone else on here had a similar experience with Evorel 75.?

Browse round.  Make notes.  Have U tried the search facility, 2 C whether there are threads regarding the HRT you are trying?

Thanks, yes I have  been reading everything I can about Evorel 75 and blood clots. Since my original post, the bleeding has become even heavier. I actually flooded in a restaurant and that was only changing after 1 hour with double protection! It was so embarrassing and we had to leave, luckily the chair was black so I didn’t  ruin it. I’m actually quite worried now as I thought Inwas post menopausal but now since starting the Evorel 75, my period has come back with a vengeance. Please please could someone tell me if passing heavy blood clots (almost like my lining is coming away, quite dark) is normal and if anyone experienced this when changing HRT? Is the oestrogen too strong or the Utrogestan not strong enough. Should I have remained on the Evorel 50 sequi (combined) and just continue to put up with the flushes? I became quite tearful last night as I feel like I’m falling apart and worried when returning to work on Monday as I’m having to change every hour 😩.
Should I contact the Doctor AGAIN, it’s so difficult to get through which is stressful in itself. I should I wait and see if it settles? I want to exercise today as K know mentally that helps but I’m so anxious about flooding.

Please please, any advice would be greatly appreciated. ☺️

Hi,  I am new to website but started on this road back in Feb 2018 when diagnosed by doctor. I was 51 then,  was prescribed Elleste which did improve my wellbeing after 2 months, but then Brexit happened and was told this HRT was hard to come by now and needed to swap.  Doctor put me on Femostan, it seemed to work ok but I did have occasional night sweats which I could cope with as long as my mood swings were under control but my periods were still very heavy and some months I could not do swimming or gym due to the heavy flow.    Then Jan/Feb 2021 my mood swings, sleepless nights, sweats started to creep back and hubby said you have to go see doc, you cannot go on like this.   I had a new doctor by then and their attitude was great, so they decided to try me on the gel and tablet combination.   So far I have been on this for 7 weeks, mood swings have improved but had a good few weeks of crying for nothing really, sweats were not every night.  But my period, well the first month I had a period after 15 days which should have happened after taking the 25th pill and then I had another period when I started taking the pills again after the 3 day break!   I thought I would wait until the next month to see if this happened again and what a surprise,  day 17 I started bleeding again not heavy as of yet but I do not know what to do now.   I read some of the posts on here about this method of HRT saying it will take a while for it to work.   The doctor said this method should stop my periods completely but now I am having double trouble hahahaha.
I have a long way to go before it all stops,  as only 54.   I try and be up beat about it as best I can but the crying outbursts is quite embarrassing especially when we are out in public.   Any advice or just to chat to someone would be good,  as there is only so much my hubby can help with hahaha.
