Title: Why is my virginal atrophy worst?

Hi beautiful ladies.
I'm 45years old, had a hysterectomy July 2018, still got my ovaries. November 2018 started getting virginal atrophy, on February 28th 2019 my doctor started me on Ovestin (a week full time then two times a week). Got 90% better I use Vagisil everyday as well. Then about late October I started getting worse with the pain. Back to my doctor mid November I started Vagifem two nights a week plus Ovestin externally two nights a week, things improved and I got 90% better again.

About a week ago I noticed my virgina (the pain for me is on the outside, sex is fine, I just feel very uncomfortable) getting worse again :'(
From Friday onwards I'm in pain all day long. I've made a doctors appointment, but can't get in until Thursday.

Why does it improve to only get worst again? I'm not allowed at the moment to go on HRT, because I was having daily ovary pains, but that has improved in the last 6 weeks.

I'm planning to ask my doctor if I can take Vagifem more often, I feel I need to do this as it's a scripted medicine, and I would need more scripts from her.

So my question is has this happened to you? And why would it improve to only go backwards? Any advice will be greatly appreciated.

Twice a week is a .minimum dose. Many women need to use it at least 5 times which the urogynaecologist told me was fine.

Hi Lyncola, I think 'that's all part of the Joy - peri/meno, the gift that keeps on giving...🙄🙄

I'm in Australia, and we don't have it, but have you thought about e-string if it's still available in the UK? I believe it needs to be fitted by your gp every three months, but then you could perhaps use the Ovestin externally as needed if the inside is ok?

I'm from Australia too, I think 'that's why it's taken me longer to get an appointment with my doctor (Australia Day Public holiday).

This premenopausal is driving me crazy.

Morning.  9.45 here.  Tell your Doctor that you will be using 'vagifem' every night again, the recommended loading is every night for 2 weeks.  Some ladies need it every night for Life.  It is a small amount and one load often isn't enough to ease symptoms.

Have a look at the 'sylc' and 'yes' web-sites, members use this on the outer labia to keep the area soft as well as either 'vagifem' or 'ovestin'.  As oestrogen levels drop so the body may become dry: inside and out - skin, scalp, legs, deep in the ears, nostrils, vagina ..........

Depends on your GP. I found it hard work getting beyond the twice a week. My practice wouldn't do it until a consultant told them to.

Hi Lyncola...When I went thru vulva HELL, pain never letting up, my GP said vagifem DAILY.  After many, many months finally getting better.  It could be that your estrogen levels are dropping further as you go along in menopause and you need vagifem and estrogen cream much more frequently than twice a week.  As well, a brilliant NP told me why vulva pain becomes worse as you use local HRT is because you are waking up tissues and nerves that are estrogen depleted.  Your vulva has tons of nerves (vagina really doesn't) so as the tissues start 'feeding' on the tiny bit of estrogen in the vagifem, it is waking up.  I would ask your GP to up vagifem at least every second night until your vulva has started to settle.  It's a long, long road but know there is a light at the end of the tunnel.

Hi Lyncola - you could always try the Ovestin cream too - I find that more effective than vagifem as you can take a bit on your finger and rub it all around xx

I actually still on Ovestin February last year, then in November got put on Vagifem as the Ovestin stopped working.

I still use Ovestin two nights a week externally.

I saw my doctor today and she said I'm having a flare up, and to use Vagifem for a week, then three times a week for three weeks, then try going back to two times a week. As well as using Ovestin externally two nights a week.

Hopefully my virgina will improve  :)

hi

i would suggest having hormones levels checked because although you have kept your ovaries these can die after hysterectomy depending on the blood supply to them and also at 45 you may well deficient in them anyway and require systemic hrt estrogen also together with the vagifem and ovestin x

I've never had my blood check for hormonal levels, my doctor does not believe in them, she says they are not accurate enough. She just goes by my symptoms that I'm premenopausal. And I'm not allowed at the moment to go on HRT, because for the first 18 months after my hysterectomy I was getting lots of ovaries pain. But I have improved with the ovary pain in the last 6 weeks, so maybe I could go on HRT in the future. At one point she started writing out the script, as she was writing it I complained about my ovaries. So that put the end to HRT. If I go another month without much ovaries problems I will discuss HRT with her again.
