Title: Tostan - Correct Dose
Post by: debra217 on September 05, 2019, 04:17:38 PM
Hello - sorry if I'm not posting this in the correct place?

I'm 2.5 months into using Evorel Conti. I've just been prescribed Tostan 1 pump 5 days out of 7 or every alternative day but reading through some of the posts on here I'm not sure if this may be too much.  Does this sound OK to start with?

Thank you for your help.

Deborah

Hi I use 1 pump 4 times a week. I think the advice from Chelsea and Westminster hospital is 1 pump every other day x

My label on my Tostran says the same as yours Debra217, but I use it as Dotty every other day. x

Please look at this thread for info on dosage and concentrations:
https://www.menopausematters.co.uk/forum/index.php/topic,39588.60.html

Also thread on personal experience with testosterone here:
https://www.menopausematters.co.uk/forum/index.php/topic,27959.0.html

See also latest guidance from British Menopause Society:
https://thebms.org.uk/publications/tools-for-clinicians/testosterone-replacement-in-menopause/

They recommend: Tostran [Kyowa Kirin Ltd] (2% testosterone gel in a canister containing 60g ) : Starting dose 1 metered pump of 0.5g = 10mg on alternate days – each canister should last 240 days.

However the Chelsea and Westminster menopause clinic recommends 2 pumps per week of 2% Tostran as a starting dose which is 20 mg and up to a max of 3 pumps (30 mg)

http://www.chelwest.nhs.uk/services/womens-health-services/gynaecology-services/menopause-and-pms-clinics/links/Testosteronereplacementforfemaleandrogendeficiency092017CWAGREED.pdf

So this dosing info is inconsistent. Therefore in your position, I would start conservatively (two pumps per week) for 3 months and see how you get on. Notwithstanding others' success with it (Dotty  ;) ), I would not want to use Tostran due to its concentration (as I am older and probably on a lower oestrogen dose) and would prefer to use the 1 % gel - now only available as testogel sachets - but as long as you start low and slow you should be fine. Also your oestrogen dose is only medium so in that scenario better to go lower to start so as not to tip the balance towards excess T?

I'm also not sure of how testosterone derived progestogens (which as norethisterone in Evorel conti) interact with replacement (exogenous= externally added) testosterone in the body? I did read something a while back re receptors and a substance called SHBG ( a compound in the body) but can't remember! Sorry that's probably not much help!

Good luck anyway and do report back.

Hurdity x

Thanks Hurdity - I will do that - 2 pumps per week for a good while and see how I get on.

As usual, superb information from this site.
