Day 1 is start of period so Day 15 is the day after mid cycle. Total cycle is 28 days although this can be academic when you are peri-menopausal. The idea is for you to have a regular bleed to shed the lining built up from the oestrogen - but in early peri-menopause especially, the two cycles can become out of sync sometimes.

Or of course if you are post meno and not having periods anymore, you can just start it on the 15th of each month, thats easiest to remember. Its supposed to mimic a monthly cycle I guess.
