Title: I got Referred - I could Cry!
Post by: heavenlyblue on September 13, 2019, 11:02:29 AM
After 3 years of awful symptoms and Evoril Conti not helping at all, I bit the bullet and asked to be referred. I was convinced they would say No.    I have seen all the GPs at my surgery, had no continuity at all, and they had little knowledge re menopause, so I just picked the most sympathetic one.  He said he had no issue referring me as I had been feeling so awful for that length of time.

Re my 36 hour migraines, he agreed with me, see if sorting out my HRT sorts out my migraines first, before referring me to Neuro.

I have been telling them all this for the past 2 years that I was sure they were linked, but no-one took any notice, just kept giving me different tables for migraines, which didn't work.  ::)

I have been referred to the Gynae Dept at my Hospital, there is about a 4-6 week waiting list.  I know you guys say a Meno Clinic is better but that was the only option and to be honest, knowing I can see someone who will see me for longer than 10 mins and actually have knowledge on what I am going through..............and it's free....I'm going to bite their hand off.  :)

If they are no help, I will then go Private.  But at least I feel I am actually getting somewhere now!   :)

Well done and what relief for you  :)

Hope you get some answers and a plan in place to suit you.
If it doesn't work out as you'd hope though, then a referral to a Menopause Specialist Clinic really can be amazingly helpful. These are also available on the NHS, but you might have to travel a bit further.

Good luck  :foryou:

Why didn't your GP refer you to a Menopause Clinic?   :-\. I would ring the Gynae secretary to make sure that the consultant is au fait with menopause!! or you will be wasting time, energy and getting no where.  Make a list of your symptoms, ring the secretary [we don't bite ;-) ] and ask the questions.  I would also ask the Practice Nurse at your Surgery if she knows how good this particularly Gynae is regarding menopause.

In the mean time, look up where your local menopause clinic is?

Why would a consultant give you more than 10 mins  :-\ - another thing to check, how long the appt. might be.  It shouldn't be such hard work but sadly  :-\

Let us know how you get on!

I do hope you get the help that you need, but I got referred to a Consultant gynaecologist with an interest in the menopause and he was useless, he believed that Vagifem could give you endometrial cancer and wanted me to have biopsies and a Mirena fitted even though I had only been using it for seven months.
I ended up going private as GP wouldn't prescribe any more HRT until I had done what consultant suggested.

Thanks for the replies, feeling a bit deflated now! But I know you ladies have been through this process and know more than me. It is appreciated.

No idea why I wasn't referred to a Meno Clinic, I asked him who I would be referred to and he said the Gynae Dept, must be their protocol. There isn't an NHS clinic one anywhere near me. They are all private.  When I get the letter through, I will get in touch with the Consultant's Secretary to get more info.

I have been in touch with a private clinic about 20 miles away, first appointment is Jan! Obviously a lot of us ladies out there not getting the help they need with the NHS. I think I should book an appointment anyway as they get booked up so far in advance, so I have it there if I need it.

It's definitely worth going ahead to see what the Gynae Clinic can offer, especially as it's a short waiting time.
It just means you've still got another route if you need it. My nearest NHS clinic was in London so it meant a bit of a journey but I felt it was worth it.
See how you get on x

I was referred to gynaecologist as the waiting list for the meno clinic was a lot longer as in gynae was 4-6 weeks and meno was well over 6 months and no definate date could be given and I needed to be seen quickly, so it may well be the same where you are too heavenlyblue x

Did you have a positive experience seeing a Gynaecologist?

I'm happy for you.

I have had similar issues - three GPs and a gynae flat out told me (and quite unpleasantly - I ran my own business for several years and would have had zero customers If I spoke to people as I was spoken to) there was no such option as HRT in gel form. "Tablets or patches, pick one."

I wasn't referred to a menopause clinic as there are none in my LHB area and they won't accept me in the nearest 60 miles away.

I hope services for women improve in the not so distant future. It's shockingly bad how we're treated (unless wealthy or in the media).

What happened to patient choice ? I thought that patients could ask to be seen anywhere these days and the Health Authorities would sort out any charges between counties  :-\

CLKD - I have no idea what happened to patient choice. I used to be a Medical Secretary for a Surgeon, but that was before Patients could decide where they were seen.

So I have a decision to make - GP from the private clinic only 10 miles away has just had an availability open up on 22 October. My gut feeling is to see her and not the Gynae (NHS) at the Hospital. If I don't take this Private date, I have to wait til Jan & it's not far to travel.

I know there is a chance the NHS Gynae  may be decent but I need my quality of life back and I just get the feeling from everything I've read and the experiences on here, that the Private Meno Clinic will offer me a more personalised service than the Gynae Dept at the Hospital.

Hi Med Sec - it was the same in 'my day' a patient went where they were referred.

What does your gut tell you?  Make that list ;-)

I had to go the sneaky way to be referred by speaking to old colleagues of mine in the NHS.

Our LHB has failed to pay what it owes to the larger LHB next door that usually takes patients that don't have access to services inside our authority. As a result, people like me are now no longer seen by the Pain Clinic. Instead we're sent on a course where we're basically told to be mindful enough and our pain and other health problems will disappear.

Not at 3am when I'm suicidal with pain they won't!

Women like me who want to see a gynae are also ignored or given antidepressants because menopause is not considered important enough to spend money on.

My neurologist is also out of my LHB. He actually told me to my face that he's treating me out of the kindness of his heart as he doesn't get paid to see me (but not in a nice way). I'm so glad my partner was with me at the time or I'd have not believed my own ears and put it down to meno brain! I really NEED to go back for treatment but have avoided it for three years as I just can't face being "treated" like that.

If I had a choice I certainly wouldn't see him again.

Sorry - rant over. It's my PMT three-week stint again.

That's what I thought. I'm taping all my consults in future as people seem to have the most bizarre reaction to me and say the weirdest things. You honestly could not make it up.

Really? If I'm only using recording to remind me what was said? I can never remember everything said at a consult - except the really bizarre stuff like, "Woman only want HRT to look more attractive to younger men".  :steamed:

Yep.  Have a chat with your local Police Station to find out what a person is/not allowed to do with recording.

Onion Relish ... there are a lot of laws regarding recording of private conversations. Not only do you need to inform the other person that you intend to record the conversation, but now you have to get their consent.
I'm not sure the Police can give you good advice, it's not their area of expertise.

I think you'll find that you can record GP consultations - just had a quick look, and it seems you don't need permission although it would be common courtesy to do so:

http://www.pulsetoday.co.uk/can-i-stop-a-patient-recording-our-consultation/20030215.article

Re data protection - the information given in the consultation is the properrty of the patient and therefore apparently data protection laws do not apply.

Data Protection is a stealth tax.  It covers areas that one would never believe and misses others  ::).  My copy ran to 72 pages and was 8" tall  ;D but referred to absolutely nothing that I was involved with.  I had to have Cover because I kept medical records on computer.

When people quote "we can't do X, Y, Z due to the Data Protection Act" I ask them to read the appropriate pages that relate to that particular conversation.  Not many have actually handled the DPA or read any of it  ::).

As for recording, yes the Police should be aware in order to give advice.  As will a Solicitor.  If one has a chaperone at the GP appt. they can also back up any queries.

My gut told me to grab that 22 Oct appointment at the private clinic only 10 miles away!    She has sent me a very detailed medical questionnaire to fill in.

Here is a link to her, Liz McCulloch,  she also works at a Hormone Clinic in Harley Street.  I have just watched the 2 You Tube interviews with her on this link, glad I could get a bit of background on her before seeing her, I like her manner :-) Fingers crossed!

CLKD did you read the links I gave - looks like neither the police nor a solicitor need to be consulted nor advise?

Good luck at your appt - this is Nick Panay's team - you are in the best hands there. However I would still go to your NHS gynae appointment.....if it occurs earlier (or anyway) and see what transpires?

Appointment is tomorrow.  Filled out the questionnaire.  Am looking forward to finally getting to see her - migraines have been awful, almost threw up on myself driving to work last week, and my whole weekend was ruined.  I don't know whether changing my HRT from Evoril Contil to body identical will improve things but I need to try something.    I would also like a hormone test, to see what is going on, and she offers three different kinds.  Have not been offered one in 3 years of seeing NHS GPs.    Also, be interested to see if she thinks I need Testosterone.  Libido and energy levels have been non existent for a long time.

I have not heard from the NHS yet about my referral.

heavenlyblue - good luck at your appointment, I'd say you made the best choice. I was referred to a gynae department and my balloon was well and truly burst when they told me they didn't deal with hrt and I'd need to be re-referred. Complete waste of time. Sounds like you've waited long enough for some sort of hrt plan and, whilst the NHS is wonderful and god forbid we lose it in this Brexit catastrophe, it sounds like they're being pushed to the limits with Menopause related issues with resultant waiting times becoming tortuous.

I'm sure your lady will have some good suggestions.

Sorry to hear about your migraines. Did you have these before starting Evorel conti and if so did they occur cyclically ie could you work out which phase of the cycle was worse? I got migraines on the conti part of this HRT patch during the first 3 months when starting HRT more than 12 years ago, and went on to bio-identcial products ( ie estradiol and progesterone) - through NHS (I am very lucky that my gynae specialist GP is very knowledgeable - not my regualr GP though!) and have been on these ever since. I do still get migraines sometimes but not that often.

I presume sumatriptan does not work for you? I find them magic but almost need to anticipate that the feeling I'm getting is going to turn into a migraine because if I leave it, the triptans don't work, but haven't wanted to take them unnecessarily every time I wake with that typical twinge starting!

All the best with your appt and do report back!

Thank you for your post.  I have suffered with them for around 20 years but they weren't that often and after a decent sleep, were gone.   I am afraid I never kept a diary so have no idea if they were cyclical or not.    But they ramped up just another level the same week my hot flushes started 3 years ago. They last 48 hours and are just awful.     I last tried Triptans about 3 years ago, they didn't work because you couldn't take them too often and I was getting them for days at a time.    They've given me Amitriptyline, Gabapentin, Topimirate...they seem convinced its nothing do with the menopause but surely they know migraines are linked to a drop in oestrogen?

Appointment went well today.    She was very nice.  She thinks my oestrogen levels are too low which could be causing the migraines  (I'm on 50mcg Evoril Conti), she wants to up it but my blood pressure was too high for her to give it to me there and then.  (I'm hoping because I couldn't find a parking spot and was panicking I'd miss the appt!   She wants me to do a 24 hour blood pressure thingy and email her the results.  So at the moment, she has prescribed me with 50mcg Evoril and 100mg Utrogestan capsules (1 a night).   Also did a blood test to see what my hormone levels are, especially testosterone, as I told her my libido had been non existent for about 4 years, she will email me the results.

So great to just have someone really listening and telling me that one size does not fit all, migraines are complex, we may have to tweak a few things....etc etc......She definitely thinks body identical Gel is the way to go for me with the Oestrogen so I can control it.    Not had one GP in 3 years do a hormone test or give me any other options apart from Evoril Conti and pain relief for migraines.      Follow up appt in 3 months but will be in touch before that when I know my testosterone levels and when my blood pressure is back down so she can up the Oestrogen.  At least I feel I am getting somewhere.  Even if it is to eliminate things.

Not sure what I will do when I get the NHS referral through though.  Not sure it will go down well if I go to the appointment and tell them I have already seen a private GP and she has prescribed me something.     May get a bit confusing if they decide on a different course of action.

Hi heavenlyblue - I've edited out the nested blue boxes with my reply as I find it gets confusing with too many (my old brain...)!

Thanks for the detailed info about your appt and also about the migraines - I gave a brief answer to your Q about sex life in the private Lives section.

I hope you manage to sort out the blood pressure issue - and also could well be white coat syndrome - mine always measures higher at the surgery. Do you know if there is any reason for it to be high? Let's hope the home readings are lower?

If you have been for a private consultation - will the consultant write to your gP with the prescription recommending what you should be given? I think that is what normally happens. When I went to a private gynae to get testosterone four years ago, she wrote to my GP and when I ran out of T (three years later) my GP was happy to prescribe it as the letter was on my file. If that is the case you may decide not to take up the NHS appt. However I do recall someone on here doing the same and the doc would subsequently only follow the recommendations of the NHS gynae not the private one as the treatments were different...just sayin' !

Anyway all the best with it all :)

Well they would always seem to take one when I was feeling awful, or anxious, or just had a 2 day migraine.  So I was never surprised I had a high reading!  Just hope when I have the monitor for 24 hours it will go back to normal.  Do not want to go on BP tablets.     She did ask me if I wanted to ask my GP for Evoril 50mcg (I'm on 50mcg Evoril Conti at the mo).  Will obviously be cheaper but I said to her I'd rather she gave me the prescription, going on past experience, I doubt they will agree as I've gone private.  I didn't want to waste anymore time.  The Utrogestan is in tomorrow but I have to wait a week for the Evoril, chemist said it takes a bit longer as its a private prescription.  I don't know if she is writing to my GP, she did ask me who it was.  So still not sure what to do when my NHS referral comes through.

Be interesting to see if I feel any different changing from synthetic to natural progesterone.      Looking forward to seeing the results of my testosterone levels.  She said she will prescribe some if it's too low.   Also looking forward to changing from a patch to gel with the Oestrogen, once my BP is down.  I feel it would be much more controllable.  NHS GPs never gave me that option.  3 years ago, I was a completely different person.  I want that person, or some semblance of that person, back!

Thank you x

She has sent me my blood test results and told me my testosterone is low.  She sent me a script for Testogel Sachets - instructions say to make one sachet last 7-10 days (30 sachets).   I will drop that into the chemist today.

Testosterone - <0.4
Progesterone - <0.2
Oestradiol -     <44

Could anyone let me know what those results tell me?

Been on Evoril Conti 50mcg the past 18 months and have had no blood test. It feels so much better to be informed.    She can't up my Oestrogen until my BP is down though, annoyingly.   But at least I have the Testogel to try. :-)

And I have gone 3 weeks without a migraine, which feels amazing!bbb    Have been following a wheat free/low sugar food plan from the Herbalist I saw recently - she also gave me a tincture to take 3 times a day.    Doing everything I can to minimise these migraines and get some energy back.  :-)

Hi Heavenlyblue

I have just read your thread with interest. Can you let me know how the Testosterone goes? I am about to have mine tested too so although I have no numbers to compare I will do soon.

Why don't you follow this thread re Testosterone.  I've just posted in there.  :-)  Chemist yesterday wouldn't order it for me without speaking to my private GP directly even thought she had the script from her.  Very frustrating.

Hi heavenlyblue - I didn't see your post in here and I answered your one yesterday about the results. As it was a separate post I forgot that I answered you on here and also forgot your story as there are so many threads and I can't keep everyone's story in my head and don't have time to scroll back through. So - I answered it more or less in isolation ie as I saw it on there. However I have now realised who you are so I have removed my post on your other thread and will repost it in the context of what you've said previously!!
