Extreme Anxiety
« on: September 09, 2024, 03:03:00 PM »

I restarted hrt in march after having 3 months off due to a heavy bleed. Biopsy showed everything ok and bleed was due to unopposed estrogen as I was experimenting with cyclogest rectally and it wasn't enough for the estrogen.

I'm now on 2 pumps of gel and utrogestan 100mg orally (I take this in the mornings). A couple of weeks ago I had a very slight bleed which was very small and stopped after a couple of hours. All in all I've been okay, my sleep has been improving, however over the last few weeks, my anxiety has started to get worse.

I'm just wanting some advice, I'm wondering if the anxiety is due to my estrogen getting out of control again.

My gynae suggested increasing my utrogestan to 200mg to stop the bleeding which I haven't done yet. I don't know whether the increase in anxiety is due to the estrogen or whether it's the utrogestan.

Thanks x

Anxiety can be caused by low oestrogen, so maybe you need more Oestrogel.

However, it can be other things too. X

Anxiety is usually low oestrogen but it can also be from too much and a few people get it from utrogestan. If you haven't been on utrogestan before it might be worth a couple of cycles of sequi to rule out utro as the cause?

Hi Disco - I was the opposite of what others said. I got anxiety when I upped my oestrogen patch and figured that, if I increased progesterone (rectal cyclogest at the time) this would address it. On a 75 patch, I then increased my progesterone to 200mg daily split into 2 doses. It helped the anxiety a lot. Only reason I came off it to go on to Livial is because I’m dealing with a couple other health issues and find it hard to manage so many different HRT components. So a little experimentation might be needed. It could be that oral uteo isn’t strong enough to balance your oestrogen, maybe tey increasing that first OR take it vaginally as it’s stronger?

Hi Everyone,

Thanks so much for the replies.

Dotty -  I really don't think it's low oestrogen. Late last year when I had the heavy bleed it was due to estrogen dominance (or unopposed estrogen, as the medical fraternity preferred to call it) so I think it might be the progesterone is not enough to counter-act the estrogen. I could be wrong, but that's what I think. Hope you're okay xx

sheila99 - My hrt ride has been a bit bumpy, as at the beginning I sort of faffed around with the utro quite a bit, so I never know whether it's the utro or not. Trying out a couple of cycles of sequi is an idea to be honest. Troubling thing is, my sleep is so much better, and if it is the utro, I really don't fancy the coil and synthetic progesterone wouldn't suit me either x

clkd - my anxiety usually begins mid to late morning; its more like feelings of dread which comes in waves until about early evening when it then eases off x

Hi Furyan - Hope your ok. I was on cyclogest last year when I got the heavy bleed. I was also using it rectally and I was cutting it as well. However when I got the bleed, my estrogen levels had shot up to about 1,600 pmol. So the bleed was just the fact I wasnt absorbing the cyclogest. My plan is to try increasing the progesterone to 200mg which is what my gynae suggested following the very slight bleed I had the other week or I may even do what sheila99 said and try a couple of cycles just to see if it is the utro which is causing my issues. I did used to use the utro vaginally at night, however i cannot seem to tolerate it at night as it seems to make me a bit wired if that makes sense x

Hi sheila99

Can I just ask doing a couple of cycles sequi. I know it means taking the utro for so many weeks, then so many weeks without the utro where you have a bleed. Can I ask how long do you take the utro for and how long do you just take the estrogen for? (I'm post menopause so I presume having a monthly bleed wouldn't cause any other issues since I had a thickened endometrium previously)

I was also wondering about whether stopping and starting the utro would cause other sort of issues as in causing my progesterone levels to shoot up and down and also I was wondering whether stopping the utro would cause any sort of withdrawal symptoms?

Sorry for the questions but my sleep is pretty much ok at the minute and I really don't want to mess that up however I cannot go on with this level of dread as well.

thanks so much x

Hi clkd

Did anything trigger your anxiety do you know?

With myself its more feelings of dread about nothing in particular which makes it seem worse if that makes sense.

I hope your okay xxx

Sequi is 200mg x 12 days (days 15-26). You carry on with your normal oestrogen all the time. They told me long term sequi gives you a slightly higher risk of endometrial cancer but I can't see it making any difference for a couple of cycles (we've done it that way for 30+ years after all). Some people get withdrawal symptoms for a few days but I don't think it's very common.
Hope you're OK CLKD, at least you have the hotel to escape to.
